The latest issue of Transfusion Alternatives in Transfusion Medicine (TATM) is now available online (http://www.nataonline.com). It is edited by Dr. Lawrence T. Goodnough from Stanford University, CA, USA, and "profiles the current state of knowledge regarding the clinical relevance of anemia in two broad settings: chronic anemias associated with patients' underlying diseases, and acute anemias in the surgical and critical care setting." The detection, evaluation and treatment of the anemia of chronic disease is associated with improved clinical outcomes. This is highlighted by Drs.

Anjay Rastogi and Allen R. Nissenson from the David Geffen School of Medicine at UCLA, Los Angeles, CA, USA, who review the evidence that shows that aggressive anemia management leads to reduced morbidity (fewer hospitalizations, improved quality of life) and even mortality in patients with chronic kidney disease.Dr. David H. Henry from the Pensylvania Hospital in Philadelphia, USA examines the evidence that for patients with cancer or cancer treatment, improved clinical outcomes include reduced exposure to blood transfusion, as well as improved quality of life. In the field of cardiology, Drs.

Donald S. Silverberg and Dov Wexler at the Department of Cardiology at the Tel Aviv Sourasky Medical Center in Israel investigate the relationship between chronic anemia and outcomes in patients with congestive heart failure.Dr. Goodnough explains that "The importance of anemia and its implications for blood management in the critical care setting are areas in which consensus is clearly needed." This is outlined by Dr. Howard L. Corwin who is Professor Of Medicine and Anesthesiology at the Dartmouth Medical School in New Hampshire, USA.

"In contrast to anemia management for patients with chronic kidney disease or cancer, no guidelines or consensus development initiatives have been put forth for patients in this setting. Similar initiatives are needed for patients with congestive heart failure" states Dr. Goodnough. "Paradoxically, for patients with acute anemias, management of anemia has been centered on the principle that substantial anemias can be safely tolerated for brief periods without undue consequences" explains Dr. Goodnough.

This is reviewed by Dr. Aryeh Shander Clinical Professor of Internal Medicine and Anesthesiology at the Mount Sinai School of Medicine in New York, USA. While Dr. Jeffrey L. Carson Richard C.
